43223Assignment: Analysis of a Pertinent Healthcare Issue
The Quadruple Aim provides broad categories of goals to pursue to maintain and improve healthcare. Within each goal are many issues that, if addressed successfully, may have a positive impact on outcomes. For example, healthcare leaders are being tasked to shift from an emphasis on disease management often provided in an acute care setting to health promotion and disease prevention delivered in primary care settings. Efforts in this area can have significant positive impacts by reducing the need for primary healthcare and by reducing the stress on the healthcare system.

Changes in the industry only serve to stress what has always been true; namely, that the healthcare field has always faced significant challenges, and that goals to improve healthcare will always involve multiple stakeholders. This should not seem surprising given the circumstances. Indeed, when a growing population needs care, there are factors involved such as the demands of providing that care and the rising costs associated with healthcare. Generally, it is not surprising that the field of healthcare is an industry facing multifaceted issues that evolve over time.

In this module’s Discussion, you reviewed some healthcare issues/stressors and selected one for further review. For this Assignment, you will consider in more detail the healthcare issue/stressor you selected. You will also review research that addresses the issue/stressor and write a white paper to your organization’s leadership that addresses the issue/stressor you selected.

To Prepare:

Review the national healthcare issues/stressors presented in the Resources and reflect on the national healthcare issue/stressor you selected for study.
Reflect on the feedback you received from your colleagues on your Discussion post for the national healthcare issue/stressor you selected.
Identify and review two additional scholarly resources (not included in the Resources for this module) that focus on change strategies implemented by healthcare organizations to address your selected national healthcare issue/stressor.
The Assignment (3-4 Pages):

Analysis of a Pertinent Healthcare Issue

Develop a 3- to 4-page paper, written to your organization’s leadership team, addressing your selected national healthcare issue/stressor and how it is impacting your work setting. Be sure to address the following:

Describe the national healthcare issue/stressor you selected and its impact on your organization. Use organizational data to quantify the impact (if necessary, seek assistance from leadership or appropriate stakeholders in your organization).
Provide a brief summary of the two articles you reviewed from outside resources on the national healthcare issue/stressor. Explain how the healthcare issue/stressor is being addressed in other organizations.
Summarize the strategies used to address the organizational impact of national healthcare issues/stressors presented in the scholarly resources you selected. Explain how they may impact your organization both positively and negatively. Be specific and provide examples.

Discussion
Our Cardiac/ Telemetry floor is one of the acute/critical care units in the hospital where I work. Nurse to patient ratios and how poor staffing can affect quality of care is one of the national healthcare issue/stressor I will talk about in this discussion. Safety of staff and patients is a major issue. When the unit is short staffed quality of care is compromised. Staff, including nurses and nursing assistants are so busy that there is no time for themselves and time to spend with patients and provide efficient care.
Nurse staffing has clearly been linked to patient safety, care quality, and cost. A commonly cited statistic is that each additional surgical patient that is added to a nurse’s workload increases the patient’s odds of dying by 7% (Aiken, Clarke, Sloane, Sochalski, & Silber, 2002). Nursing workload is increasing, and many traditional workload measurement systems, such as nurse to occupied bed ratios, diagnosis-related group (DRG) models, and intensity (timed task-based) models, do not take into account the substantial non patient-focused workload that impacts nursing today. (Swiger et al, 2016)
One of the goals implemented by the ACA’s triple aim is enhancement of patient care experience which includes quality and satisfaction of the patient (Marshall & Broome, 2017). One thing my unit is currently doing to improve quality care is by offering incentives of additional $20 per hour on top of our regular hourly rate if you pick an extra shift, and nurses from other units with same acuity of care are also encouraged to pick shifts for these incentive to help solve the problem of shortages in staff. The incentive idea so far has helped with the nurse-patient ratio from 5 patients per shift to 4 patients, there by giving nurses to have more time for themselves and to the patients. According to our unit Director’s reports from patient’s survey, patients are more satisfied than before. However, I think the unit will face the same problem when the incentive project is over because it has been initiated just for a short period of time. But again there is plan to hire and train more staffs before they stop the incentives; hopefully this helps.
Staffing for patients with varying severity of symptoms is critical to safety and quality outcomes. Staffing must be flexible to match staff competence with patient needs (American Psychiatric Nurses Association, 2105). This is so true because often times we have patients who are suicidal, confused and are pulling things off them like NG tubes and need to be on a 1:1 precaution and usually it is always an issue to get extra help for this. Again and issue the hospital is trying to solve by offering incentives and hiring more staff.
