This assignment is intended to help you learn to do the following: 

  • Interpret interrelationships between theory, philosophy, practice, and research in professional nursing.
  • Simulate the application of nursing theory and how it informs clinical judgment and decision-making in providing safe, effective nursing care.

Overview

This module’s material has addressed multiple nursing theories and theorists. This assignment requires you to role play, responding to a situation by applying your selected theory as though you are that theorist. You are to use the same theory/theorist for all assignments in this module.

Before you begin this assignment it is recommended that you complete the required research in the assignment Nursing Theory Academic Paper.

Theorist Role Play

How would the theorist you selected apply his/her theoretical concepts in providing nursing care in the following situation? Assume that nursing care will be provided throughout a course of three months, with the varying phases as described in the story.

You are the nurse at Hospice Caring, a private hospice service in your community. You first meet Stephan Froelich, an Austrian immigrant to the United States as he becomes enrolled in hospice, in May of this year. Stephan, age 64, has terminal lung cancer, diagnosed 8 months ago. This is a recurrence of cancer, for which he had been successfully treated 10 years ago. This time however, the cancer had metastasized throughout his body before he was diagnosed, and he chose to forego treatment, wanting to spend his last days with his family and friends, without the side effects of chemotherapy, surgery, and radiation.

Stephan’s family includes his wife of 42 years, JoAnna, his three daughters, Mary, Elizabeth, and Patricia, and his five grandchildren, ranging in age from 8-14. He is especially close with his grandson Corey, age 10, who lives nearby and who shares his love of fishing. The Froelich’s are very involved in their church, and have been leaders of their congregation.

Upon your first visit with Stephan, you talk with him about his expectations, assess him, and speak with his family members about their concerns. Stephan tells you, “I have a strong faith, and I know the Lord is waiting for me. I am more worried about my family, especially my wife. We’ve been together since we were kids, and she will have a hard time.” He says he wants to remain in control, and is worried that the pain will get too bad, and cause him to have to take drugs that make him sleepy. He asks if there is anything he can tell his grandson Corey, and wants advice about how much to say to him.

JoAnna is tearful, but has accepted Stephan’s desire to die at home, and expresses her desire to help him stay comfortable. She has many questions about medication for pain, concern about his poor appetite, and wonders whether he will get sick from the many visitors that have come to see him.

Over the next few months, as you continue with your visits with Stephan and his family, you glean insight about their fears, their strengths, and areas of concern.

By June, Stephan has become bed-ridden, getting up only to go to the bathroom, and to sit at the table for meals. He has little appetite, but enjoys the company of his family. Members of their church have set up a rotation schedule to bring meals to the family, and so there is always a lot of company, visitors, and friends. Corey has been able to see his grandpa during the week, now that school is out for the summer. Stephan’s daughters are able to visit frequently, and express concern to you about their mother, who they say is very tired, does not sleep well, and who has not been away from Stephan’s side for several weeks.

In early July, you observe that Stephan is sleeping most of the day, but is lucid when he is awake. He has begun to read the Bible more often, and likes to have JoAnna and their church friends read passages to him when they visit. He does not eat much food, but is able to drink milkshakes and take supplements. He is not able to get out of bed, even to go to the bathroom, and his very embarrassed to have anyone assist him with toileting. There is a bedside commode, but he is too weak to get up by himself to use it. He has asked you to talk with him about what to expect, and wonders, “How much more time do I have?” He says the pain is “ok” but you notice he seems uncomfortable at times. He does not want to take narcotics, as he says they make him “dopey”. JoAnna has focused on trying to get him to eat, and asks for help with his diet.

By late July, Stephan’s health has begun to deteriorate markedly. He is only awake for a few moments at a time, takes only a few sips of liquid each day, and sometimes does not recognize friends and family members. When he talks, he does not seem to understand where he is. You have been administering pain medication, and he seems to be comfortable. You have begun to focus quite a bit of attention on JoAnna and other family members. They need support as the end comes near, and you are preparing them for what to expect in the last few days.

On the morning of August 10th, JoAnna calls you and says that Stephan is only breathing a couple of times each minute. You arrive just as he is taking his last few breaths. His whole family, the minister, and a few close friends have gathered at his bedside.

    • As the theorist you are role-playing in this assignment, how would you respond to the situation presented in the Overview?
    • What would be the voice of your selected theorist? Keep in mind that you are role-playing that person. Use the vocabulary of and demonstrate the concerns and perspective of that theorist in your response.
    • Which key concept(s) does the theory/theorist reflect (person, health, environment, nursing)?
    • How does that influence your response? How did your research inform you and influence how you understand this theory/theorist? (See the Nursing Theory Academic Paper assignment). This reflection does not require you to write anything. However, your reflection will impact what you write in the action item below.
  4. By the due date indicated, write a 150-200 word response to the situation presented in the Overview as though you are the theorist you selected. Post your response to the Discuss topic area, Nursing Theory Discussion. Your response is to be written in the first person. Think about it as if you are the theorist and you are responding to a nurse who presents you with this situation. What would you say? How would you say it? Your response is to include the following:
    • State the name of the theorist/theory.
    • Explain in 1-2 sentences the major concepts/components of your theory.
    • Describe what you would do in this situation and why you would do this. Base your answers (what and why) on your required reading and the scholarly research you have completed for the Nursing Theory Academic Paper.
