Sketch- a- solution assignments must clearly connect proposed solutions to the theory or model studied that week. This presentation assignment needs to include solutions for each level of the social-ecological model and how the individual and community can be influenced to support maternal child health outcomes, specifically include ideas based on social cognitive theory and social capital theory.

Why is this part called a presentation? You are being asked to refine content from your ideas list to showcase what you’ve learned from this module. Your grade will be based on your ability to clearly identify theoretically-based solutions linked to course content that could support pedestrian safety and public health.

Directions:

Step 1: Review Crazy 8s ideas posted in the discussion forum. Review your own list and up to four other lists submitted by your peers to help generate more ideas. 

CAUTION! Avoid looking at too many lists to prevent idea overload. There will likely be more than 100 ideas submitted and self-control with idea absorbing will be essential.

Step 2: Refine your original ideas, looking for solutions you think need improvement or clarification.

You can use other peers' ideas to refine your list.

Remember you will need at least one solution for each level of the social-ecological model.

For example:

Individual-level: your personal behaviors that can support new solutions

Group and Family level: friends, family, classmates, academic advisors, professor, anyone who supports public health solutions

Organizational level: The university, place of work, or place of worship/spiritual practice, a healthcare system that can support the solutions

Societal level: your city, state, or country and things they can do to support the solutions

Policy level: courses of action on a governmental level that can support the solutions

Include ideas linked to constructs in the following theories:

social cognitive theory

social capital theory

Step 3: Review & refine again:

Try looking at the solutions from different points of view: as a health professional, a patient, parent, a program participant, a funding agency, a loved one.

Repeat this process as needed until you have solutions you are satisfied with.

Step 4: Define & detail:

Use ideas and strategies you are confident about. Use what you absorbed from the learning materials and assigned readings. Be certain your ideas include correct information and cite your sources. Adjust and erase ideas or let them naturally fade away as you add more detail to the solution.

Remember to connect it to the content of the class! Failure to clearly connect each solution with the theory or model of the unit will result in a 10 point deduction for this assignment. Have fun with this assignment, but keep it relevant to the course!

Week 4: Part 2 Sketch a Solution

Once upon a time… John and Jane were talking about starting a family. However, they were concerned about Jane’s current level of health with regards to pregnancy. Jane decided to schedule an appointment with her doctor to address her concerns. (Social-Ecological Model, Individual)

Every day… John took time to ask Jane what her goals were or the day and how he could help her to accomplish them. Jane really appreciated John’s willingness to support her. (Social Capital Theory, Bonding)

One day… Jane woke up with nausea. Three positive pregnancy tests confirmed their suspicions. Along with John’s help, Jane has been consistent with the doctor’s recommendations on diet and physical activity. Now that they have a new baby to take care of, Jane is motivated to stay diligent. This gives her confidence that she can continue her new/modified behaviors to ensure a healthy pregnancy. (Social Cognitive Theory, Self-efficacy)

Because of that… Jane’s doctor had new recommendations for her during a routine pregnancy check-up. Jane’s physical appearance is proof that she has been consistent with her health behaviors. The doctor recommends Jane seek out a health and wellness coach to help prescribe a specific diet and physical activity prescription for her specific needs. This will also provide her with a role model to assist her through the process. (Social Cognitive Theory, Behavioral Capability)

Because of that… Jane starts to feel more confident in her ability to regulate her health for positive quality of life outcomes. Jane has a lot of social support from not just John but close friends and neighbors as well. Jane decides to use her new knowledge and motivation, along with her lost love for dance, and begins hosting a dance workout group in her home twice a month. Her close friends, family, and neighbors are all supportive in one way or another. (Social Cognitive Theory, Reciprocal Determinism)

Until finally… the big day has arrived! Jane’s water broke and John is taking her to the hospital. Jane has lost a considerable amount of weight. John and Jane’s new and modified health behaviors were their norm now and they were confident that Jane’s healthy pregnancy would lead to an uncomplicated delivery. Jane gave birth and quickly shed the pregnancy weight off since modifying her lifestyle. Neighbors are asking Jane if she can provide workout sessions to the community through the homeowner’s association (HOA). The president said he would allow the use of the community recreation room to conduct these workouts and they would be promoted through the monthly community newsletter. CJ – Dreaming of Becoming of Mother

CJ, 25, has been gaining weight over the last five years and wants to start a family with her spouse and become pregnant with her first child in the next year. CJ met with her doctor and her doctor said that shee needs to incorporate more physical activity into her daily routine. CJ is a medium height person, weighing 200 pounds, and really does not know how to be physically active on her own.

CJ’s experience with physical activity was somewhat limited in variety. 6 months ago, she read that being within a healthy weight range could reduce pregnancy and delivery complications. After that, CJ started walking in her neighborhood for 15 minutes, once a week. CJ would go for walks by herself, sometimes her spouse would join her and she seemed to enjoy that. When CJ was 18, she went to a dance class every week, for 45 minutes and she would see other friends at the class, they were good at the dance class. The dance class was cancelled and CJ moved away and has not danced or done any kind of group physical activity since then.

CJ wants to be more active for the sole reason that she wants to have a healthy pregnancy and reduce the chance for infant or maternal death during delivery. CJ saved $50 dollars to buy anything that will help her be physically active. CJ has access to the Internet, but is not familiar with where to search for good health information regarding physical activity and maternal & child health. CJ is employed but does not have access to an employee health program, instead she receives a monthly newsletter with words of encouragement to be healthy.

CJ generally makes friends easily, but has been isolating due to COVID-19 pandemic. She has expressed wanting to be phsycially active to friends through text messages. Her friends have tried to be supportive and said they would cheer her on if she started some kind of homebased phsycial activity routine. CJ plans to talk to her doctor next week about what kind of exercise is appropriate.

The Problem

Big picture: Heart Disease is the leading cause of death in Arizona. Heart disease consists of many type of issues related to the function. In this case study, we focus on heart health issues related to lifestyle behaviors (eating, exercising, stress reduction, sleep, etc.) 65% (4 million people -enough to fill 66 NFL stadiums), of Arizona adults reported being overweight, 10% reported having diabetes, and 5% reported having heart disease.

Maternal Mortality is the death of women while pregnant or within 42 days of terminiation of pregnancy due to any cause, related to or aggravated by, the pregnancy or its management but not from accidental or incidental causes. According to the Centers for Disease Control (CDC) the top leading causes of maternal deaths in the U.S. are: cardiovascular disease; other non-medical cardiovascular conditions (lifestyle conditions i.e. overweight, shortness of breath, smoking tobacco, substance use, excessive stressful habits), infection, hemorrhages (bleeding) Arizona State Health Assessment, 2019).

Maternal deaths related to childbirth in the U.S. have increase steadily since the CDC began tracking pregnancy related deaths, rising from 7.2 per 100,000 in 1987 to 18.0 in 2014 (CDC Division of Reproductive Health). Another concern related to maternal deaths are the complications that can happen during pregnancy, labor, and delivery. Some effects are mild and some are severe and range from high blood pressure, respiratory distress, amniotic fluid embolism, eclampsia, heart failure during a procedure, severe anesthesia complications, infection, shock, and blood transfusion problems to name a few.

Some of the consequences of the complications include unexpected pregnancy, derlivey, and postpartum complications like bleeding, organ failure, and stroke. The complications not only affect the health of women, but also their fetuses who may suffer low brith weight, premature brith, or death. Severe complications in pregnancy has been slowly increasing in recent year and affected more than 50,000 women, last counted in 2014. It is not clear why sever complications are increasing, but changes in overll health of the population of women giving birth may be contributing to increases in complications.

While maternal mortality in Arizona remains below the national average (20.3 out of 100,000 live births, during 2012-2016, compared to 26.4 nationally), Arizona has also seen increasing rates (ADHS Bureau of Vital Records).
