Throughout your career in health and human services (HHS), you will have opportunities to engage with patients and their families in various healthcare settings (for example, hospitals, private practice, outpatient facilities) to develop a care plan for managing their chronic health ailments and illnesses. It is important to master essential skills, such as time management, active listening, and effective communication, when in the process of creating a care plan for your future patients.

For Task 2, you will apply these skills in a real-world scenario using the simulation platform Mursion. During this simulation experience, you will interact with a client and a family member or members (avatars voiced by an actor), with the goal of agreeing on a care plan for the client. You can read more about Mursion in the attached “Mursion Simulation Guide.”

To prepare for this simulation, select a scenario (Scenario A or Scenario B), and read the scenario description and attachments for information about the situation and the characters who will be part of your simulation experience. You will use this information to create a plan that will help you during the simulation to focus on the main objectives of Task 2. You will submit your task, including the care plan and reflection, but you do not submit the simulation recording. The recording is for your reference since you must use specific examples from the simulation in your plan and reflection.

Scenario
Select one of the attached scenarios (“Scenario A: Cardiology Rehabilitation Client” or “Scenario B: Behavioral Health Client”) to be the focus of your Mursion simulation experience and performance task for Task 2. Read the scenario and follow the preparation instructions for only one scenario.

Preparation for the Simulation

Prior to meeting with the client and family member or members, you will develop a plan that involves the perspectives of the client, family, and healthcare professionals involved in the client’s care using the provided scenario and documentation. Use the SHARE model to draft an initial plan that you will take with you to the simulation meeting. This plan is an initial draft that you will modify during and after the meeting to incorporate the client’s and family’s perspectives. Address the following in your plan:

1. Describe three services the client will need in the care plan.

a. Identify three professionals that could be involved in the care plan.

b. Describe one specific contribution that each of the three professionals would offer to the care plan.

2. Explain how two potential socioeconomic or physical barriers may hinder the client’s access to the services.

a. Identify one strategy to overcome each of the two potential barriers.

3. Explain how you will follow up with the client.

Also, prepare a communication plan for the simulation meeting with the client and family member or members by doing the following:

1. Prepare to resolve the difference of opinion between the client and family member or members regarding the client’s care.

2. Prepare communication techniques that will be used when meeting with the client and family member or members to gain their input and support of the plan

After completing the simulation, revise your initial care plan as needed to incorporate learnings from your meeting with the client and family member or members.

Complete the Mursion simulation using the information provided for your selected scenario, and then do the following:

A. Develop a care plan in collaboration with your client and family by doing the following:

1. Describe three services the client will need, using specific examples from the scenario and the simulation experience.

a. Identify three professionals that could be involved in the care plan.

b. Describe a specific contribution that each of the three professionals in part A1a would provide in the care plan, using details from the scenario.

2. Explain how two potential socioeconomic or physical barriers may hinder the client’s access to the services.

a. Describe a strategy to overcome each of the two potential barriers, using specific examples from the scenario and the simulation experience.

3. Explain how you will follow up with the client.

B. Write a reflection about your experience with the collaborative development of the care plan by doing the following:

1. Explain how your initial draft of the care plan changed after your interaction with the client and family member or members, using specific examples from the simulation to support the changes. If the initial draft did not change, explain how it was collaboratively agreed upon among all parties.

2. Explain how you followed each step of the SHARE model in your final draft of the care plan, using specific examples from the simulation.

a. Describe one challenge you faced in collaboratively developing the care plan and how you responded to the challenge, using specific examples from the simulation.

3. Describe what you did well to collaborate with the client and family member or members, using specific details from the simulation experience.

4. Describe what you would have done differently in your interactions with the client and family member or members, using specific details from the simulation experience.

C. Acknowledge sources, using in-text citations and references, for content that is quoted, paraphrased, or summarized.

D. Demonstrate professional communication in the content and presentation of your submission.
