One possible variation where trauma may factor is in dissociative disorders. Although dissociative disorders are not categorized within trauma- and stressor-related disorders, psychological trauma can trigger dissociative disorders and those suffering from trauma- and stressor-related disorders can encounter dissociative symptoms, such as flashbacks.
Thus, diagnosing in this area requires careful attention. In this Assignment, you practice diagnosing in these areas.

· Remember: When using Z codes, stay focused on the psychosocial and environmental impact on the client within the last 12 months.

· The diagnosis should appear on one line in the following order. Note: Do not include the plus sign in your diagnosis. Instead, write the indicated items next to each other.
Code + Name + Specifier (appears on its own first line) Z code (appears on its own line next with its name written next to the code)
Then, in 1–2 pages, respond to the following:
· Explain how you support the diagnosis by specifically identifying the criteria from the case study.
· Describe in detail how the client’s symptoms match up with the specific diagnostic criteria for the disorder (or all the disorders) that you finally selected for the client. You do not need to repeat the diagnostic code in the explanation.
· Identify the differential diagnosis you considered.
· Explain why you excluded this diagnosis/diagnoses.
· Explain the specific factors of culture that are or may be relevant to the case and the diagnosis, which may include the cultural concepts of distress.
· Explain why you chose the Z codes you have for this client.

CASE of HUONG
INTAKE DATE: July xxxx
IDENTIFYING/DEMOGRAPHIC DATA: Huong is 48-year-old, widowed Vietnamese female. Huong immigrated from Vietnam 21 years ago with her husband. They have 4 children who are all currently out of the house. Huong lives in San Jose, California.
CHIEF COMPLAINT/PRESENTING PROBLEM: Huong presented to the emergency room reporting that she had overdosed a handful of antihistamines shortly before she arrived. Huong lost her husband 2 ½ years ago. She described missing her husband terribly since they did everything together. They had a very active life in the United States and fondly remembers all their activities since arriving in the United States. She feels betrayed by his passing and has no interest in life any longer feeling so alone after having devoted much of her 28-year marriage to being a wife, mother, and homemaker. After clearing Huong, the hospital referred her to outpatient treatment.
HISTORY OF PRESENT ILLNESS: Huong reports being sad and tearful at times, and she occasionally has difficulty sleeping. She feels desperate and suicidal when she thinks “he is gone, this is not supposed to happen this way.”
PAST PSYCHIATRIC HISTORY: Huong reports no previous psychiatric difficulties.
SUBSTANCE USE HISTORY: Huong denies any drug or alcohol use.
PAST MEDICAL HISTORY: Huong reports being in good health. She has not had any major illnesses.
CURRENT FAMILY ISSUES AND DYNAMICS: Huong loved being a wife and mother. Her family is “her world”. They enjoyed time with family and friends together. Huong’s childhood was unremarkable. She married her husband in Vietnam and he was offered a job in the United States working in technology, so they immigrated to the U.S. The move was challenging at first since she did not speak the language but once the children started school she acculturated to the American way. They had a variety of friends and Huong accommodated her husband to be successful in his career. She was involved in many activities at the children’s school where she met a wide friendship circle. She reported no difficulties in her marriage. Her mood has varied these past two years; she admitted to anxiety and depression.
MENTAL STATUS EXAM: Huong is a well-dressed woman who looks her stated age. Her mood is down, and she lacks eye contact. Her affect is anxious. Motor activity is appropriate. Speech is clear. Thoughts are logical and organized. There is no evidence of delusions or hallucinations. On formal mental status examination, Huong is found to be oriented to three spheres. When asked how she sees her future, she responded, “there is no future”.
