You will create an informational fact sheet that educates a vulnerable population on the prevention, detection, and treatment of a disease or health condition.

Consider this scenario: You work at a county health department and are asked to create educational material for a vulnerable population in your area that addresses the detection, prevention, and treatment of a disease or condition. You will determine how the Social Ecological Model can address a health issue based on the information you have read about through the Healthy People 2030 goals and objectives. As a health educator, how would you create material that is most successful in helping vulnerable populations in your own community understand how to better their health?
To prepare for this Assessment:
Select one Healthy People 2030 topic area and one or more objectives to address directly.

For example, if you choose to address the “Food Safety” objective, there are 6 different categories with 1–10 objectives associated. You can choose to address any of these objectives that apply.
Identify one disease or condition that relates directly to the topic area and objectives to concentrate on.
Research statistics on the disease or condition in your area, in surrounding areas, and nationally. You may also choose to locate global statistics.
Research at least two programs that have been used to address the disease or conditions.
Select a model or theory from the resources provided that coincides with one of the five hierarchical levels in the Social Ecological Model. Reflect on how the model or theory explains a health condition associated with the Healthy People topic area previously chosen.
Create a 1-page (front and back) information fact sheet to be distributed at various clinics and doctor’s offices that will help to educate the vulnerable population on the disease or condition.
The brochure should address prevention, detection, and treatment of the health condition or disease. Use the following fact sheets as examples to guide you:

Describe the Healthy People 2030 topic area and objectives that you have chosen and how they apply to your chosen health condition or disease.
Apply at least one level of the Social Ecological Model to help explain the health behaviors associated with the health condition or disease.
Apply at least one theory or model associated with the selected Social Ecological Model level(s) that helps to explain the health condition or disease.
Examples of theories and models include the Health Belief Model, Transtheoretical Model, and Theory of Planned Behavior, among others. Address prevention, detection, and treatment of the health condition or disease within your brochure.
Include at least two different statistics that are pertinent to the selected vulnerable population and area, and offer some type of comparison (local, state, national, or global).
Your fact sheet should offer information that is culturally and linguistically sensitive, specifically to the vulnerable population that you identified as being particularly affected by the disease or condition.
Your brochure/fact sheet should be visually appealing. Include at least two pictures, graphics, charts, or other such materials that are culturally appropriate.
